What potassium actually does

Potassium is the main positively charged ion inside your cells, and nearly every tissue depends on keeping it within a fairly narrow range. But potassium has two more specific jobs that make it directly relevant to blood pressure.

The first is in the kidneys. Your kidneys are the body's sodium thermostat, and one of the ways they fine-tune that thermostat is by swapping sodium and potassium as they filter blood and build urine. When more potassium arrives, the kidneys tend to push more sodium out into the urine rather than holding on to it. The classic observational evidence comes from INTERSALT, a study of more than 10,000 adults across 52 centers in 32 countries that measured sodium and potassium not by asking people what they ate, but by collecting their urine over a full 24 hours. After correcting for the measurement error that blurs these kinds of estimates, the researchers found that a 50 mmol/day higher potassium excretion — roughly 2,000 mg — was associated with about 3.4 mmHg lower systolic pressure. Sodium pointed the opposite way, as expected. The study couldn't prove cause and effect on its own, but its scale and its objective measurement made it a landmark in how researchers think about the sodium-potassium pair.

The trial evidence followed. A widely cited 1997 meta-analysis published in JAMA pooled 33 randomized controlled trials of oral potassium supplementation and found an average drop of about 3.1 mmHg systolic and 2.0 mmHg diastolic across all participants — with a larger effect, around 4.4 mmHg systolic, in people who already had high blood pressure. A more recent 2013 systematic review and meta-analysis in the BMJ reached a similar conclusion: increasing potassium intake reduced systolic pressure by about 3.5 mmHg in adults with hypertension, with little measurable effect in people whose pressure was already normal.

Those numbers deserve a careful reading, because "3 to 4 mmHg" can sound too small to bother with. It isn't. At the population level, a shift of even 2 mmHg in average systolic pressure is associated with a measurable drop in stroke and heart-attack rates — which is why public-health bodies take these findings seriously enough to write them into dietary guidance. At the individual level, a few points can mean the difference between a number that sits just above a treatment threshold and one that sits just below it. It's not a replacement for anything your doctor prescribes, but it's also not nothing.

One more finding is worth keeping in mind. The same 2013 BMJ review found that, in the observational studies, higher potassium intake was associated with roughly a 24% lower risk of stroke. The sodium-to-potassium ratio

The practical translation is encouraging rather than burdensome. Because the same swaps that cut sodium usually add potassium at the same time, moving the ratio in the right direction doesn't require a second, parallel diet. It requires one change of direction, repeated. A meal built around beans and vegetables instead of deli meat and bread improves the ratio from both ends simultaneously.

That said, the ratio is a lens for understanding, not a number to track obsessively. You can't see it on a food label, and chasing it with an app is a fast way to turn a simple idea into a chore. It's most useful as a reminder that the goal was never "eat nothing salty." It's "move the balance," and the balance moves when the foods on your plate change.

What about supplements?

Here the story takes a sharp turn, and it's worth being clear about it: potassium is one mineral where the supplement aisle is genuinely not the shortcut it appears to be.

In the United States, over-the-counter potassium pills are capped at 99 mg per dose — a tiny fraction of the 2,600 to 3,400 mg target. That cap exists for a reason. Decades ago, higher-dose potassium chloride tablets in a wax-matrix form were linked to lesions in the small intestine, and the Food and Drug Administration responded by limiting what could be sold without a prescription. The rule is codified in federal regulations, and it's why a bottle of "high-potency" potassium from the drugstore is almost always just 99 mg per pill.

To put that in perspective: a single banana contains more than four of those pills' worth of potassium. You'd need roughly nine of them to match one baked potato. Trying to reach a meaningful daily intake through over-the-counter pills is expensive, tedious, and pointless when food does the job better and more safely.

That's not to say high-dose potassium doesn't have a legitimate medical role — it does. When someone genuinely needs large amounts, it's available as a prescription drug, carefully dosed and monitored because getting it wrong can be dangerous. The rule that matters is simple and applies to most dietary advice: make changes gradually, and if you have a condition or a medication that affects potassium, ask your doctor before you make them at all.

One exception worth repeating: If you have kidney disease, or you take a potassium-sparing diuretic, an ACE inhibitor, or an ARB, do not increase your potassium intake — especially from supplements — without talking to your doctor first. For everyone else, food sources are safe and worth adding.

Practical swaps that actually stick

The reason most people under-consume potassium isn't ignorance of bananas. It's that their default meals are built from low-potassium, high-sodium building blocks, and changing that means changing the defaults.
